I'm definitely of the "orgasm while inflated is dramatic" school of thought. I would lean to the increased gasping, muscle straining, and waves of pleasure- being a great tax on the body.
In many fantasies- depending on how inflated, this would just lead to bursting. But if feeling durable enough- just a stretching surge of growth- that leads to a nice slow deflation as everything relaxes.

Personally, I think it's a question like "is it safe to be very worried/excited when inflating?" If it's in the same line of thought, then real life translates as follows:
* i.s. with inflatee at utmost limits inevitably leads to popping
* i.s. with only slightly inflated person instantly deflates him/her
* i.s. with someone inflated between the extremes is fun and has no effect on inflation
And since rough i.s. could decrease size in future inflations and no inflator/inflatee would want that - i.s. would always be very careful and delicate ^_^
