Yeah most are just amazed at how the costume inflates and have fun with it. An actress will encapsulate the role and play it as you want. But like others said it's just a role and scene to them. The inflation is just a funny side bonus that they can laugh at because it's just plain ridiculous.
The only way they could tell it's fetishy minus prior knowledge is if the script is a little too keyworded or if you as the director are starting to throw off signs of arousal (women can tell).
But yeah nothing special they act just like anyone else.

Not to take the fun out of this, but we're quite a niche, so the majority of people wouldn't assume inflation is also a fetish. An actor/actress is just a job, so they'd do whatever needed for the script they agreed to.
When Annasophia Robb played Violet in 2005, she said it was fun to blow up and get big and blue, but she was still quite immature and a child, so she would have just thought it was funny.
