As gross as it is, this is a pretty good question. Unfortunately, I have no practical answer, as I have never experienced this while inflated. I also don't know anyone who could (or would) share such an experience. I would hope if there is such an individual on here, they would shed some light on the topic at least by PMing you.
My first instinct is to say "yes, some bloating may occur, though probably not noticably so." Bloating associated with diarrhea is caused by gasses expanding in the bowels. Even if the bowels are already inflated, the gas is still gonna be produced, and it has to go somewhere. However, given that (from personal experience) the amount of gas produced like this is relatively small comapred to inflation, and during such times there is little in terms of outward appearence, I'd expect there to be almost no visual change. I can only foresee the usual painful cramping being somewhat more intense and harder to disapate.
Really, the only way I can think of to actually find out for sure would be to have the experience. I hope this has been somewhat useful.
